Assessment Request

Submit an assessment request for a refinance, an actual property sale, or another property-related transaction. Requests are processed within 48 business hours.

1. Select One Request Type

Clearly identify whether this is a refinance only, an actual sale with an ownership change, or another type of request.

2. Assessment Month

The assessment is valid only for the calendar month of the requested closing or transaction date.

3. Report Changes

If the sale does not close or the closing date changes, contact WFWRD at 385-468-6325.

Important: This assessment is valid only for the calendar month containing the requested closing or transaction date. If the sale does not go through, the closing date moves to another month, or any transaction details change, contact WFWRD at 385-468-6325 so the account can be updated.

What is this assessment request for?

Select one: Choose Refinance only when ownership will not change. Choose Actual sale when the home or property is being sold and ownership will change. Choose Other for raw land or any request that is not a refinance or an actual sale.
Enter the first name of the property owner refinancing the property.
Enter the last name of the property owner refinancing the property.
Explain the type of request and provide any information WFWRD will need to complete the assessment. Raw-land requests must be entered here.
If the parcel number is unknown, enter zeros.
Service Address or Property Location
The assessment will be valid only for the calendar month containing this date. Contact WFWRD if this date changes or the transaction does not occur.
Enter the seller's full name.
Enter the seller's email address if available.
Enter the seller's phone number if available.
Enter the buyer's full name.
Enter the buyer's email address if available.
Enter the buyer's phone number if available.
Include any additional information WFWRD should know about this request.
