This is a subscription service for eligible residential addresses participating in curbside glass recycling.
Service includes a $55 start-up fee and $8.50 monthly billing.
Glass recycling is collected once per month at participating addresses.
Important: Complete all information carefully. The account must be current before glass recycling service can be added.
This form must be submitted by the homeowner. The name on this request must match the deed or property ownership record for the home. If you are a renter or tenant, please have the homeowner or landlord submit this form.

This agreement is entered into between the property account holder and Wasatch Front Waste and Recycling District for the provision of a Glass Recycling can. By submitting this request, the account holder acknowledges and agrees to the following terms:
